Fat Transfer for Penis

Fat grafting to the penis, sometimes referred to as a fat transfer phalloplasty, is an effective way for men to achieve penile girth enhancement. In a process known as autologous fat transfer, a surgeon harvests excess adipose tissue from a donor area on the patient’s body, treats it, then reintroduces the fat into the desired area for increased, natural-looking volume. What is Autologous Fat Transfer?

Autologous fat transfer is a way to increase penis girth, or thickness, without the use of fillers or prostheses. Research data indicates that fat injection is the most commonly used technique to increase penile size. (1) Using liposuction, or specially designed syringes, excess fat is harvested from a predetermined area of the body. The most common donor areas for fat transfer are the abdomen or love handles. After the right amount of fat is harvested, it is filtered in an atraumatic manner to preserve fat cell viability  After filtration, the fat cells are injected into the tissue of the penis for increased tissue volume in the shaft. Platelet rich plasma (PRP) is then added to the fat to improve its survivability. The combination of fat transfer along with platelets is called S.T.I.F. (Super Thrombocyte Injections with Fat).

One of the key benefits of this method is that there is no foreign body reaction from the immune system, so adverse reactions are rare. This is because the fat comes from the patient’s own tissues. It’s also far less invasive procedure compared to other more involved penis augmentation methods. (2)

Candidates

Candidates for fat transfer to the penis are healthy men with no urogenital diseases or serious medical conditions. The best candidates have realistic perceptions of their current size, and realistic expectations of fat transfer results. Fat transfer is not suitable for patients who have:

  • A history of keloid scarring
  • A blood clotting disorder
  • A severe chronic disease
  • A known hypersensitivity to anesthesia
  • Unrealistic expectations

Your personal consultation provides Dr. Neavin with valuable personal information. No detail is off-limits, so feel free to be frank about the issues you are experiencing. Be prepared to provide your medical history, a list of medications you currently take, and the main reasons why you want the surgery. In an examination room, Dr. Neavin will perform a physical exam and note the current length, stretched length, and circumference (girth) of your penis. After he has gathered all of the relevant information, he will explain how a fat transfer procedure can benefit you. And, if you’re a suitable candidate, he will schedule your treatment date.

Preparation

  • Follow all of your preoperative instructions carefully.
  • Stop smoking in the weeks before your procedure.
  • Stop taking blood-thinning medications (such as aspirin).
  • Stop taking blood-thinning supplements (such as ginkgo biloba)
  • Arrange a ride to and from our location with a trusted adult.
  • Fill your prescriptions a few days before your surgery
  • Prepare your living space for recovery (pillows, entertainment, etc.)

Procedure

Dr. Neavin will administer a regional nerve block before harvesting fat from the donor site outlined in your treatment plan. The adipose tissue is then placed in syringes and spun in a centrifuge for purification. The forces exerted on the tissue separates it into three layers; an oily layer, an adipose layer, and a lower aqueous layer. The uppermost and lowermost layers are discarded and the middle adipose layer is collected. Once a local numbing agent has been applied to the penis, the purified fat is ready for grafting. Dr. Neavin places the purified fat into the shaft of the penis via a small incision at the base, between the erectile tissue and the skin. Once the fat transfer process is complete, he will close the incision and wrap the penis in a protective bandage.

Recovery

After your procedure, you will need a few days to rest and recover at home. Though fat transfer recovery is more straightforward than recovery from surgeries with multiple incisions, it’s still important not to push too hard to resume your regular activities. As is expected after any surgical procedure, there will be some residual swelling and discomfort which can be managed with your prescribed medication.

  • Keep the penis wrapped with self-adherent bandage until otherwise directed.
  • Take pain medication exactly as prescribed.
  • Continue to refrain from taking blood-thinning medications.
  • Do not smoke until you are given the O.K. to do so.
  • Refrain from sexual intercourse for 6 weeks.
  • Avoid strenuous activities and wait before resuming exercise regimen.
  • Perform penis stretching exercises to counter clumping.

Results

You will see preliminary results immediately, however, the grafted fat will need time to settle. Although a small percentage of the grafted fat will be reabsorbed by the body over time, a lot is retained in the penis. Most patients can expect to see their final results in 2 months.

The S.T.I.M. shot is an injection of platelet rich plasma (PRP) to enhance penis size and sensation.
